Does having temperature up to 38 C mean ALWAYS a sign of immune system fighting?

If one has occasional temperatures up to 38 C and it goes away itself, does it mean the body's immune system is fighting agaisnt something, such as possible infections? should it be ignored if it does not persist or go beyond 38 C?

i was taught exactly that in nursing school, and at the hospital we can't give anything for the fever unless it gets up to 38 C (100.4F)
